Further Casting Info Request

I realized unfortunately that I'd included my standard audition form in the big Packet...and it's missing several key pieces of information that'll factor into casting. 

*sigh*

Please take some time to go through the questions in the link below and respond.  Your full honesty would be deeply appreciated...I don't want to put folk in a position where they have a chance to be anything but massively successful.
